Individualized CARE for OLDer persons with complex chronic conditions in home care and nursing homes I-CARE4OLD

I-CARE4OLD (Individualized CARE for OLDer persons with complex chronic conditions in home care and nursing homes) has the overall aim to improve prognoses and estimation of treatment impact for older care recipients with complex chronic conditions in home care and nursing homes settings. Moreover, I-CARE4OLD aims to develop, validate, and test next generation decision support using high quality internationally standardised routine care data based on the comprehensive assessment tool InterRAI.

The I-CARE4OLD multidisciplinary international consortium will (1) link high quality and structured routine care assessments of functional capacities, diseases, and treatments with administrative repositories on mortality and care use of home care and nursing home sites on 52 million patients from eight countries; (2) develop and validate prognostic algorithms using a variety of techniques including artificial intelligence such as machine learning to better predict (i) outcomes and the modifying impact of (ii) pharmacological and (iii) non-pharmacological treatments; (3) test user experiences of health professionals on individual patients with an e-platform that processes algorithms on new assessments real time.

I-CARE4OLD was granted 60 MSEK and will involve 10 countries, including Israel, the USA and CANADA. Karolinska Institutet will receive 3 MSEK.
